Insurance Information
We hope that the below information is helpful in answering your questions
about insurance coverage.
Dr. Stone is currently on the Provider List with Blue Cross PPO and
Blue Shield PPO.
All other PPO insurance will be accepted as a non participating
provider. This means that your insurance may only pay a partial
payment, even after your deductible or co-pay. You will be responsible
for any balance that the insurance does not provide payment for.
Because of the wide variety of insurance plan coverage benefits
available, various co-payments, deductibles, and percentages of
coverage may or may not apply to your particular insurance plan.
Therefore, all co-payments, deductibles, and patient responsible
percentages are due and payable at time of visit. If you have any
questions regarding your insurance benefits you may want to call
your insurance member services department. The telephone number
is usually listed on the front or back of your insurance identification
card.

We are also accepting assignment with Medicare. We will bill Medicare
for you.
If you have a deductible or percentage that Medicare indicates as
your responsibility, such amount will be due and payable at the
time of your visit. However, you may have a secondary PPO insurance
plan coverage that may cover the amount that Medicare does not pay.
In that circumstance, we will bill that insurance plan for you.
If you do not have insurance, please note that payment for services
will be due at the time of your visit.
At this time, we do not accept HMO Insurance Plans, Medi-Cal,
or Medi-Aide.
